Falls, Orthopaedic and General Rehabilitation

Our Therapy service see patients within the Wakefield district who require assessment, treatment and rehabilitation services. Rehabilitation can be provided in any community setting including patients own homes, residential care homes, residential rehabilitation setting.

About us

The aims of the service include: 

  • To provide individual rehabilitation at home after a hospital discharge. This could be supporting you to return to your usual function after a period of acute illness, planned surgery or fracture recovery. 

  • To assesses falls risks within the home environment and provide suitable home -based exercise and advice to support falls prevention. 

  • If you have a deteriorating condition or are a palliative gold line patient and experiencing changes with your mobility or managing activities around the home, we may be able to support by completing an at home assessment and providing appropriate equipment and aids

The services are-split across the Wakefield district into three bases: Central Wakefield, North East and South East. 

We aim to triage all referrals with the patient themselves in-order to gain an understanding of their goals. If you do not meet the MY Therapy criteria at this time, we may be able to signpost you to other services in the area which are able to meet your needs/ wishes. 

Urgent Community Response to hospital admission avoidance

These referrals would be prioritised to complete a timely assessment to support patients to remain at home if it is safe to do so, and prevent unrequired admission to an acute hospital. 

My Therapy work closely with Adult Social Care, Integrated Care Team and District Nurses to facilitate this. 

My Therapy also work closely with Virtual Ward. This is a multidisciplinary team approach, with Adult Community Nursing Services, to support short term monitoring of medical deterioration in the community. This may support discharge from hospital or to prevent an admission if able.

MY Therapy statistics

MY Therapy have been running a walking recycling scheme and have so far recycled 213 pairs of crutches, 97 walking sticks and 32 zimmer frames along with other equipment that can be used again reducing waste.
