Single Point of Contact (SPOC) in Wakefield, is a point of contact for adults (over 18) registered with a Wakefield GP who are using our community health services. Or for all referrers, families or carers supporting people using our services.

Single Point of Contact (SPOC) is a point of contact for people with a Wakefield GP using our services. This is a single telephone number but we endeavour to prioritise calls that require an urgent response or people, their family or carers particularly those at the end of their life.
