About us

Cardiac rehabilitation provides vital support and information to help you in your recovery and includes:

  • How to return to activities and exercise safely
  • Improve your lifestyle
  • Live well with your condition
  • Achieve your health and wellbeing goals

All members of the Cardiac Rehab team will work in partnership with you and your family to develop a plan that is safe and suitable for you.

We aim to empower patients with the skills and knowledge to manage their long-term health conditions confidently.

We can offer you a choice of options for your personalised cardiac rehab programme.

The Cardiac Rehabilitation Service consists of Specialist Nurses and Physiotherapists supported by Secretarial staff. We accept referrals for people who are registered with either a Wakefield District or Dewsbury (North Kirklees) District GP.

We will contact patients following patient discharge or from when we receive your referral. An initial assessment appointment will then be arranged,  either via home visit, telephone, video or clinic.

Conditions we treat

  • Diagnosis of a heart attack (Myocardial Infarction).
  • Patients who have had coronary artery bypass graft surgery or heart valve surgery.
  • Patients who have had percutaneous coronary intervention (PCI), sometimes known as angioplasty.
