At Mid Yorkshire, our specialist bereavement midwives are committed to providing emotional and practical support to families who have suffered the loss of a baby in pregnancy or following birth. They can support you and your family to organise funerals, register deaths, and signpost you for ongoing support.

In tragic situations such as this, everyone’s needs are very different so we will work with families to provide sensitive and compassionate care that is tailored to their needs - both individually, and as a family.

The following anonymous questionnaire is for individuals who have experienced the loss of a baby, or babies, during pregnancy or shortly after birth.

We invite you to reflect on any aspects of the care you received before, during, or after the birth of your baby, highlighting what was important to you, your partner, and your family.

Please complete as much of the form as you feel comfortable with, ticking the relevant boxes and adding any comments you may have. We understand that some questions may not be applicable to your experience, so please answer only those that are relevant to you.

Forget-me-not yearly service

Each year, on the last Sunday in November, we hold a service in Pinderfields' chapel to remember families who have experienced miscarriage, stillbirth and child loss. It is an opportunity to share together in a simple act of worship and remembrance, regardless of faith.
