Community nursing for children

The Children's Community Nursing Service is made up of four specialist nursing teams, all of whom provide specialist nursing care to children and young people across the Wakefield District. The service is mainly provided within the child's own home or in a community setting.
The teams are:
- Children's Community Nursing Team (CCN) – offers holistic nursing support to children and young people with specific health needs.
- Jigsaw Team (Children's Palliative Care) – offers holistic nursing care to children with life-limiting or life-threatening illness.
- Children with Learning Disabilities Nursing Team (CTLD) – offers holistic care for children with a learning disability and additional healthcare needs.
- Children's Continuing Care Team (CCCT) – provides direct packages of care to children with continuing healthcare needs.
The service also provides specialist nursing equipment for children with additional healthcare needs.
The service is provided mainly within the child's own home, but may also be within other areas accessed by the child such as in schools, short break units and children's centres, depending on the needs of the child. Our office is based 20-22 Bar Lane, Wakefield.
We offer this service Monday to Friday, from 9am to 5pm. Additional support is also available during acute periods and for end of life care where required.
The Continuing Care team operates over a 24-hour period, depending on the type of care planned for individual children.
Children are referred to the teams via partner agencies such as health and social care professionals.
